<p>Week one is mostly about getting comfortable. The first session is the longest, around 45 minutes for the depression protocol, because we map your motor threshold, which tells us exactly how much energy your brain needs.</p>
<p>That first visit matters because we are not guessing. We find the stimulation level your brain reliably responds to, then build the rest of the course around that. Good mapping makes the later sessions more consistent and takes some of the mystery out of the process.</p>
<p>You'll feel a tapping on the side of your head. Not pain, more like a woodpecker on a hard hat. The first few sessions can be slightly uncomfortable while your scalp gets used to it. By day three or four, most patients describe it as boring.</p>
<p>A little scalp tenderness or a mild headache early on can happen. Most people still finish the session, drive themselves home, and go back to work or school. There is no anesthesia, no recovery room, and no foggy rest of the day.</p>
<p>You probably won't feel different yet. That's normal. Your brain is being introduced to something new. It hasn't had time to respond.</p>
<p>This is where people sometimes get discouraged too early. TMS is not a single chemical event like taking a pill. It is repeated stimulation of a network that has been underfiring for a long time. Week one is usually setup, not payoff.</p>
<h2 id="when-the-lift-usually-starts">When the lift usually starts</h2>
<p>Weeks two and three are where things get interesting. Patients describe it the same way over and over. <em>Something is lighter. I don't know what.</em></p>
<p>The first sign is rarely a mood change. It's usually a sleep change. People fall asleep easier. They wake up less. The fog around mornings starts to thin.</p>
<p>Then the smaller things show up. You laugh at a podcast in the car. You text somebody back you've been avoiding. You eat lunch and actually taste it.</p>
<p>Sometimes the first person to notice is not you. It is your spouse saying you're less flat at dinner. It is your roommate noticing you came out of the bedroom without forcing yourself. It is a parent hearing more life in your voice on the phone. Those are real changes, even if you would not call yourself better yet.</p>
<p>This is your dorsolateral prefrontal cortex coming back online. The part of your brain that handles motivation and emotional regulation has been undergunned for a long time. TMS is reminding it how to fire.</p>
<p>The lift also does not always happen in a straight line. Two better days and one flat day is still progress. We look for trend, not perfection. If you're waiting for a movie-scene moment where everything suddenly feels bright, you can miss the quieter but very real early gains.</p>
<h2 id="why-six-weeks-not-two">Why six weeks, not two</h2>
<p>You'll see clinics online quoting different timelines. Three weeks. Five days. Some are real protocols. Some are aggressive marketing. The standard course cleared by the FDA for major depressive disorder is six weeks of daily sessions, Monday through Friday, followed by a two-week taper.</p>
<p>There's a reason for that. The brain doesn't change in three days. Neuroplasticity, the way neurons reshape their connections, takes repetition. Each session lays down a tiny piece of new wiring. Six weeks gives the brain enough reps to make the change stick.</p>
<p>Think of it less like taking a pain pill and more like doing rehab on a weak circuit. One session matters, but the stack of sessions is what changes the system. That is also why missed appointments are not ideal. You want the course to build on itself.</p>
<p>When patients try to cut the course short because they're feeling better, the gains often slip back. We always recommend finishing.</p>
<p>The taper matters too. Once the brain starts responding, we do not like to go from full support to nothing overnight. Spacing sessions out at the end lets us see whether the lift holds as the schedule loosens.</p>

<h2 id="what-counts-as-a-real-response">What counts as a real response</h2>
<p>The clinical word is response. If your depression score on the standard tool, the PHQ-9, drops by 50 percent or more, you've responded. If it drops to a level that says you're no longer clinically depressed, you've remitted.</p>
<p>Different studies show different numbers, but the broad picture is consistent. Around 60 to 65 percent of patients respond to TMS. Around 30 to 40 percent fully remit. These numbers are higher than what you see for someone trying their third or fourth medication after the first two failed.</p>
<p>The score matters because it keeps us honest. Depression is persuasive. On a bad day, it will tell you that nothing is changing. A weekly measure gives us something sturdier than mood in the moment.</p>
<p>Real-world response often looks like this:</p>
<ul><li>You can think about your future without dread sitting on your chest</li><li>The cycle of bad sleep stops feeding the bad days</li><li>You enjoy something specific again. A song. A plate of food. A walk.</li><li>Therapy starts working better because you have enough fuel to actually use it</li></ul>
<p>We also care about function. Are you showering without a full internal fight. Are you answering work messages. Are you less irritable with the people closest to you. Are you spending fewer hours pinned under the day. Those changes count.</p>
<p>That last one is underrated. TMS doesn't replace therapy. It clears the static so the therapy can land.</p>

<p>The worst way to measure TMS is to ask yourself every night, <em>Am I fixed yet?</em> Most people cannot answer that clearly. A better question is, <em>What got a little easier this week than it was two weeks ago?</em></p>
<p>Progress often shows up in ordinary places first. You start the dishwasher instead of staring at it. You make the phone call you kept putting off. You sit through church, class, or a staff meeting without feeling like you have to escape your own skin.</p>
<ul><li>You get out of bed with less argument</li><li>You lose fewer hours to rumination</li><li>You tolerate other people better</li><li>You have enough focus to use what therapy is teaching you</li></ul>
<p>We tell patients to let one trusted person watch with them. Partners, parents, and close friends often notice the return of humor, eye contact, patience, or follow-through before the patient is ready to believe the change themselves.</p>

<h2 id="what-can-speed-it-up-or-slow-it-down">What can speed it up or slow it down</h2>
<p>Nothing makes the brain obey a perfect deadline, but some things make the course cleaner. Consistency matters. Five weekdays in a row works differently than scattering treatment across cancellations and long gaps.</p>
<p>Sleep matters too. If someone is barely sleeping, the depression picture gets noisier and harder to treat. The same goes for big mid-course medication changes unless there is a clear clinical reason. When too many variables change at once, it gets harder to tell what is helping.</p>
<p>We also keep the broader clinical picture in view. Depression is not always traveling alone. Trauma symptoms, heavy substance use, untreated medical problems, or a diagnosis that needs a second look can all make the timeline feel slower or less straightforward. That does not mean TMS cannot help. It means the plan has to match the actual person in front of us.</p>

<h2 id="when-tms-doesn-t-move-on-schedule">When TMS doesn't move on schedule</h2>
<p>Sometimes you hit week four and you don't feel much yet. That doesn't mean it isn't working. About 20 percent of responders are late responders. They get most of the lift in week five or six, or even in the maintenance phase.</p>
<p>Late response is frustrating because the calendar says you should be farther along than you feel. This is exactly when some patients want to quit. We usually tell them not to make that call too early, because the people who respond late often look flat right up until they don't.</p>
<p>If you're in week six and there's been no movement at all, we have other moves. We can extend the course, change the target site on the brain, or pivot back to a medication conversation. We don't pretend a treatment worked when it didn't.</p>
<p>We also review the basics instead of just repeating the same session and hoping. Was attendance consistent. Has sleep collapsed. Is depression the whole story here. Good care means asking those questions before calling something a failure.</p>

<p>At <a href="/">RSLNT Wellness</a>, we run TMS the way the research supports. Five days a week for six weeks, then a two-week taper. We track your response with weekly PHQ-9 scores, not vibes.</p>
<p>We also track function, because numbers are only part of the picture. We ask about sleep, concentration, work, appetite, irritability, and whether the people around you can feel the difference. If the score is improving but life is not opening up, we take that seriously.</p>
<p><strong>Counseling alongside TMS.</strong> We pair TMS with talk therapy whenever possible. Patients who do both move further than patients who do TMS alone. Cognitive behavioral therapy or acceptance and commitment therapy works especially well during the lift. <p><strong>Medication management.</strong> If you're already on an SSRI like sertraline or escitalopram, we don't take you off it during TMS unless there's a reason. Many patients keep their medication, and we adjust or taper later as the brain stabilizes.</p>
<p><strong><a href="/how-tms-works">TMS therapy</a>.</strong> Sessions are about 19 minutes once we're past the first week. You can drive yourself to and from the appointment. No anesthesia, no needles, no recovery time. Patients often go straight back to work or school.</p>
