<p>You didn't decide to start pretending. You drifted into it. Probably starting in adolescence, when you figured out that adults responded better to "I'm fine" than to "actually, I'm not."</p>
<p>The mask became automatic. By the time it started costing you, it was running so deep you couldn't tell where the performance ended and you began.</p>
<p>This pattern has names in the literature. Researchers call it emotional labor when it's expected at work, and masking when it's a daily strategy in personal life. A 2023 American Psychological Association report found that workers in service-heavy roles, parents of small children, and people with high-functioning depression all show elevated rates of physical exhaustion linked to sustained masking.</p>
<p>You're not lazy. You're tired. There's a difference.</p>
<p>For some people, the act started in a house where being easy was the safest role. For others, it started in school, church, sports, medicine, sales, or a family where someone else's crisis took up all the oxygen. You learn quickly which feelings get welcomed and which ones make the room colder.</p>
<p>That adaptation can look impressive from the outside. Good grades. Good work ethic. Reliable friend. The person who never needs much. A defense can get rewarded so thoroughly that it starts passing as personality. Then one day the honest sentence finally shows up: I'm exhausted from pretending I'm okay.</p>

<h2 id="what-the-body-is-actually-doing">What the body is actually doing</h2>

<p>Pretending to be fine when you're not isn't free. The body charges interest.</p>
<p>When you suppress an emotion in real time, your sympathetic nervous system stays activated. Heart rate stays up. Cortisol stays elevated. Digestion slows. Your prefrontal cortex spends bandwidth on impression management instead of actual thinking.</p>
<p>Do this for years and the bill comes due in unexpected ways. Migraines. Stomach problems. Insomnia at 3 a.m. Cravings for sugar or alcohol or scrolling. A short fuse with the people you love most.</p>
<p>The cost isn't only mental. It's physical. The body is keeping score.</p>
<p>Clinically, we hear about jaw clenching, neck pain, shallow breathing, chest tightness, and a brain that goes blank after a long day of being "on." None of that is random. Your system is spending energy keeping the outside calm while the inside stays braced.</p>
<p>People often assume they need a vacation, a supplement, or better time management. Sometimes those help around the edges. But if the core problem is chronic self-suppression, you can organize your calendar perfectly and still feel wrung out by dinner.</p>

<h2 id="you-re-not-faking-the-smile-you-re-forgetting-the-inside">You're not faking the smile. You're forgetting the inside.</h2>

<p>Here's the part that scares people. The longer you pretend, the harder it gets to find the real feeling underneath.</p>
<p>Patients describe it like this. <em>I don't know what I want. I don't know what I'm sad about. I don't know if I'm sad. I just feel blank when nobody's looking.</em></p>
<p>That's not numbness for no reason. That's what happens when your inner world has been on mute long enough that you stopped checking it. The signal isn't gone. It's just on a frequency you can't pick up anymore.</p>
<p>A clinician's first job, often, is to help you remember how to listen.</p>
<p>A lot of high-functioning adults can identify everybody else's mood before they can identify their own. They can tell you which kid is struggling, which coworker is irritated, which parent is disappointed. Ask what they feel and the answer is often a long pause.</p>
<p>That pause matters. It tells us access has been interrupted, not that you have no depth. Therapy often starts with questions that sound simple on purpose. What happened in your chest when that text came in? What did you want to say and not say? What do you feel when the house finally gets quiet?</p>

<h2 id="why-high-functioning-doesn-t-mean-you-re-fine">Why high-functioning doesn't mean you're fine</h2>

<p>High-functioning is one of the more misleading labels in mental health. It usually means you are still meeting obligations. It does not mean you feel rested, connected, or safe inside your own life.</p>
<p>We see the nurse who gets through a 12-hour shift and cries in the car before driving home. The business owner who closes the deal and then stares at the ceiling at 2 a.m. The parent who packs lunches, answers emails, makes the game, and still feels absent from their own day. Competence and suffering can coexist for a long time.</p>
<p>Home is often where the mask costs the most. Not because you love your family less. Because home is the one place your nervous system stops performing long enough for the irritability, shutdown, or tears to show up. That is often the moment people realize this is not a personality quirk. It's depletion.</p>

<h2 id="what-putting-down-the-act-actually-means">What "putting down the act" actually means</h2>

<p>People worry that if they stop pretending, they'll fall apart. They picture crying at work, snapping at their kids, telling the truth at dinner parties, getting fired.</p>
<p>That's not how this goes.</p>
<p>Putting down the act means you stop performing for an internal audience that isn't even watching. It means you tell the truth in one safe place, your therapist's office, and let the rest of your life keep functioning. The world outside doesn't have to know you're doing the work. They just have to notice that you seem a little less tired.</p>
<p>Therapy is a private, contained space where the mask can come off without consequence.</p>
<p>The first honest sentence in therapy is rarely dramatic. It's usually something like, <em>I'm handling everything and I still feel dead tired</em> or <em>I'm the dependable one and I resent everybody</em>. Good therapy can hold that without turning you into a crisis.</p>
<p>You do not have to confess your whole life in week one. You start where the truth is close enough to reach. Then you build the capacity to feel what is actually there without immediately shoving it back down.</p>

<h2 id="how-to-start-without-anyone-noticing">How to start without anyone noticing</h2>

<p>If you can't picture telling anyone you're starting therapy, you don't have to.</p>
<p>You can:</p>
<ul><li>Schedule a video session during your lunch break</li><li>Tell your spouse you're "talking to someone for stress" without specifying more</li><li>Use a dedicated email account if you're worried about privacy</li><li>Pay out of pocket if you don't want anything on insurance records (we offer this)</li></ul>
<p>Your treatment is yours. The performance can keep going until you're ready to put it down. By that point, you'll have tools. The tools are the freedom.</p>
<p>A small starting point helps. Notice the three moments in a normal day when the act gets heaviest. Before work. On the drive home. When someone asks how you are and you answer too quickly. Write down what your body does in those moments. Not your whole story. Just the pattern.</p>
<p>You can also choose one person who has earned a slightly more honest answer. Not a dramatic disclosure. Just less polish than usual. <em>I've been more worn down than I look.</em> For many patients, that is the beginning of real relief.</p>

<h2 id="when-this-needs-more-than-self-discipline">When this needs more than self-discipline</h2>

<p>Sometimes people try to solve this with stricter routines, harder workouts, better supplements, or another productivity system. If the problem is ordinary stress, those can help. If the problem is depression, anxiety, trauma, grief, or a nervous system that has been running hot for years, self-discipline alone usually becomes one more performance.</p>
<p>Pay attention if the exhaustion comes with waking dread, panic, loss of interest, emotional flatness, drinking just to come down, snapping at people you care about, or thoughts that everyone would be better off without you. Those are not character flaws. Those are signs to get evaluated.</p>
<p>Good evaluation also keeps medical humility. Thyroid problems, anemia, sleep apnea, chronic pain, medication side effects, postpartum changes, and perimenopausal changes can all intensify emotional exhaustion. A serious clinician does not wave those away and call everything anxiety.</p>

<h2 id="how-we-actually-treat-this-at-rslnt">How we actually treat this at RSLNT</h2>

<p>The first step is not proving you're sick enough. It is figuring out what is underneath the performance. For one person it's untreated depression. For another it's trauma. For another it's years of being the reliable one in a family system that never made room for their needs.</p>
<p><strong>Counseling that doesn't require you to fall apart first.</strong> Our clinicians use cognitive behavioral therapy and acceptance and commitment therapy. ACT is especially useful for chronic masking, because it teaches you to relate to feelings without having to perform them or suppress them. We also work with internal family systems for patients whose protective parts have been running the show for decades.</p>
<p>CBT helps us identify the rules underneath the act. Don't burden people. Don't need anything. Stay useful. ACT helps you stop organizing your life around avoiding discomfort. Internal family systems can be useful when a protector part has been working overtime for so long that you barely remember what it was protecting.</p>
<p><strong>Medication management for the chemistry under the mask.</strong> SSRIs like sertraline and escitalopram, SNRIs like venlafaxine, and bupropion when motivation is the issue. We adjust based on how you respond. We don't push pills. We don't withhold them either.</p>
<p>If medication is appropriate, we talk through expected benefits, common side effects, sleep impact, sexual side effects, and how long to give a medication before calling it a fit or a miss. Function matters, but so does feeling like yourself.</p>
<p><strong><a href="/how-tms-works">TMS therapy</a> when high-functioning depression has worn the engine out.</strong> TMS is FDA-cleared and uses gentle magnetic pulses to wake up the part of your brain that handles mood. It's drug-free, with no recovery time. Patients who have been masking for years often respond well, because the underlying flatness has been neurochemical the whole time.</p>
<p>For the right patient, TMS is done outpatient over a series of sessions and does not require sedation. It is not the answer for everybody. But when someone has been doing all the right things and still feels emotionally blunted, it can be a reasonable next step.</p>
<p>Some patients need weekly therapy only. Some need therapy plus medication. Some need a sleep evaluation, a primary care workup, or couples work because the mask has shaped the whole household. Treatment is not one-size-fits-all. The point is to reduce the strain of performing your life.</p> <aside class="rslnt-cta">
